This Self-Care Survey was developed by Tina Boogren in her book Take Time for You: Self-Care Action Plans for Educators. The questions are correlated with Maslow's Hierarchy of Needs. 

Activity created for a student at Summer Institute 2012. Opposites Board is used to have the student sort the items in their corresponding sections. Skill: Sorting Age range: 3-8

Lori Chambers put these files together for students to check in during Summer Institute 2012. Files are used for making worksheets for individual students, as well as materials to put together a large 5 Point Scale Checkin for the classroom. Skill: Emotional Regulation Age range: 2-21
