FERPA Consent Form Spanish- APH Federal Quota Registration

The federal Act to Promote the Education of the Blind mandates an annual Federal Registration of all legally blind students. The Kansas Department of Education, which is responsible for the accurate reporting of Kansas’s students, has designated the Kansas Instructional Resource Center for the Blind and Visually Impaired (KIRC) to conduct the statewide census. The number of eligible legally blind students registered with the American Printing House for the Blind (APH) determines the federal allocation to the Kansas State Department of Education's Federal Quota Account. The fund is used to purchase educational materials from the American Printing House for the Blind for eligible registered students. Registration occurs in January of each year.

“FERPA Consent Form Spanish” is a release form that is required to be signed by a parent or guardian before a student can be registered for APH Federal Quota Funds. FERPA forms must be kept on file by the TSVI or LEA, and are good until the student exits the district or program. This consent form ONLY allows for KIRC to share basic registering information with the American Printing House once a year- only the registration team at APH can see the data, and after verification the data is randomized before it is used or released to any other entity. Their data is also wiped each year before the next registration window begins. If you need to register a new student, please add them through WebOrder or email kirc@kansasblind.gov
